The Forma is a resource used to change or add a Polarity slot to a weapon or frame, or used to add Tiles onto a Clan Dojo, or used in blueprints for clan weapons.
Acquisition[]
Forma or its blueprint can be obtained in one of the following ways:
- Purchased in the market for File:Platinumicon.png 20.
- Received as a log in reward.
- Alert Rewards.
- As a mission reward from a Void Mission.

Using Forma on an Equipment[]
Forma can only be used on equipment that has already been ranked to 30. When it is used on anything, its rank is completely reset (to unranked). You can use it again on already polarized equipment, provided you leveled it to 30 again. Your Orokin Catalyst or Orokin Reactor will remain in the Item after using a Forma.
Just as selling and re-leveling a piece of equipment, re-leveling a newly polarized piece of gear still does not grant mastery points (5/24/13).
Exactly as it says in game when you go to apply Forma to an item: "Customize this item by Polarizing ONE of the Mod Slots Above (click a slot to cycle Polarity). Choosing Apply will RESET ITEM RANK! Any installed super-charge will remain."
